介護支援専門員実務研修受講試験に合格し、実務研修を受け、各都道府県の介護支援専門員名簿に登録することで資格が取得できる。受講試験を受けるには、福祉、保健、医療に関する指定された法定資格を持ち、対人援助業務の経験が5年以上かつ従事した日数が900日以上であること、あるいは特定の福祉施設、介護施設、障害者支援での相談援助業務の経験が5年以上かつ従事した日数が900日以上であることなど、一定の資格や実務経験が必要である。資格登録後は、介護保険事業所として指定を受けた居宅介護支援事業所（ケアプラン作成機関）、あるいは介護保険施設に就職し業務を行う。
ケアマネジャーになるのは、介護福祉士や社会福祉士など介護や相談援助の仕事を経験してきた人や看護師が多い。
福祉や医療に関する幅広い知識、介護保険をはじめとする福祉・医療関連の様々な制度や施策への理解が必要である。また、介護者を側面的にサポートする対人援助の専門家として、介護を必要とする高齢者や家族など、ケアサービスを受ける立場に立って考える姿勢はもとより、サービス利用者のプライバシーへの配慮、守秘義務や高いレベルの倫理観などが求められる。
ケアマネジャーの資格は更新制となっており、5年ごとに更新に必要な研修を受け、申請手続を行う必要がある。
ケアマネジャーの上級職として主任ケアマネジャー（主任介護支援専門員）制度も導入されている。主任ケアマネジャーはケアマネジャーを統括し、指導・育成を行うほか、経験が必要なケースを担当する。主任ケアマネジャーになるには、ケアマネジャーとして5年以上の経験があり、主任介護支援専門員研修を受講する必要がある。
